Upon an employee’s death, the provident fund and other benefits that are due to the deceased should be initiated with the company’s HR manager.
HR must provide Phylis with all the forms, so she can submit her claim to the provident fund administrator.
She’ll be provided with a UI-19 form to submit at the labour department.
